Course overview

In most businesses, a control failure costs money. In oil and gas, chemicals, mining, aviation, construction and energy, a control failure can kill people and destroy the company. That difference reshapes everything: the controls are layered, the assumptions are challenged, and the reporting of a near miss is treated as valuable rather than embarrassing.

This course covers risk in high-hazard industries across five units: understanding high-risk industries, hazard identification and assessment, mitigation and control strategies, safety management systems and compliance, and crisis preparedness and resilience. It is educational and does not certify participants or provide regulatory approval of any kind.

What major accident investigations keep finding

The same things. Warning signs present and normalized. Maintenance deferred under cost pressure. Alarms disabled because they went off too often. Procedures that no longer described how the work was actually done. A safety case that was written once and never revisited. And a culture in which raising a concern was a career risk.

Almost never a single unforeseeable event. The Swiss cheese model holds up depressingly well: the barriers were there, and the holes lined up because each one had been quietly enlarged over years.

Course objectives

By the end of the course, participants will be able to:

  • Sequence hazard scenarios from causes to the top event.
  • Select engineering, administrative and protective equipment controls.

  • Recognize organizational drift that precedes major accidents.

Unit 1: Understanding high-risk industries

  • Characteristics of high-hazard operations.
  • Major accident hazards versus occupational safety.
  • Documented major accidents and organizational failures.
  • The regulatory landscape and the duty holder's obligations.

Unit 2: Hazard identification and risk assessment

  • Structured techniques such as hazard and operability study.
  • Bow-tie analysis: preventive and mitigating barriers.
  • Failure mode and effects analysis, fault and event trees.
  • Quantitative risk assessment: individual and societal risk.

Unit 3: Risk mitigation and control strategies

  • The hierarchy of control: elimination and substitution.
  • Layers of protection and independent protection layers.
  • Barrier management: identifying critical barriers.
  • Permit to work, isolation and management of change.

Unit 4: Safety management systems and compliance

  • Safety management system elements: policy and planning.
  • ISO 45001 and process safety management frameworks.
  • Safety cases and their ongoing maintenance.
  • Leading and lagging indicators and their limits.

Unit 5: Crisis preparedness and organizational resilience

  • Emergency response planning and command structure.
  • Coordination with emergency services and regulators.
  • Incident investigation: root cause and recommendations.
  • Safety culture: near-miss reports and organizational drift.

How the course is delivered

The course works from published accident investigations, bow-tie diagrams, safety cases and inspection findings. Participants analyze the barriers, identify where they degraded, and argue the control decisions in discussion. There is no field or plant environment. The course is educational: it does not certify participants, does not approve any safety case, and is not a substitute for the competence assurance your regulator requires.
